1299 Puerperal Shoulder Pain: A Rare Case of Glenohumeral Joint Septic Arthritis

Abstract Introduction Native glenohumeral joint septic arthritis is a rare manifestation of an occult bacteraemia. Without urgent diagnosis and intervention, can lead to rapid cartilaginous destruction. We report unusual presentation postpartum streptococcal arthritis. Case Description A fit healthy 34-year-old woman gravida 2 para 3 developed unremitting shoulder pain, reduced range movement tachycardia day 1 after vaginal delivery associated with grade two perineal tear. Initial blood work revealed rising inflammatory markers, CRP 275, WCC 16.2. radiographs showed no significant abnormality. T2 weighted MRI large effusion. All samples from aspirate grew Group B streptococcus (GBS), concordant high swab taken in the late third trimester. The patient underwent arthroscopic washout synovial sampling, growing GBS. Transthoracic echocardiogram imaging evidence infective endocarditis. was commenced on appropriate 6-week intravenous antibiotic course clinically improved. Physiotherapy rehabilitation improved movements. Discussion There scarcity orthopaedic complications reported obstetric pathology. Atraumatic pain physiological, or biochemical signs infection should alert diagnosing surgeon consider Haematogenous bacterial translocation cause atypical presentations Routine parameters arthrocentesis continues prove be gold standard diagnosis. compilation case series detailing location, microorganism, complications, effective treatment would useful tailor management these presentations.
